This collection contains scanned facsimiles of manuscripts held in the collection of the Historical Medical Library of the College of Physicians of Philadelphia, reproduced here with the Library's permission. Details regarding the original collection are available at the Historical Medical Library's website.

The majority of these letters deal with the staffing and management of Mount Sinai's Out-Door Department, the hospital dispensary, which Jacobi oversaw as Chairman of the Supervising Committee. The collection includes a copy of the Department's rules and regulations from the early 1880s. The collection also contains two letters regarding Jacobi's resignation as Attending Physician and his advancement to the newly created position of Consulting Physician.

of Mt Sinai Hospital
The undersigned, a Committee to confer with the Medical Staff about closing the Dispensary on certain
days, respectfully report,
That at a meeting held between this Committee and Drs. Jacobi, Herzog and Munde on the evening of
the 13th inst., the question was thoroughly discussed and it was

made apparent to the undersigned,
1st That any difference in arranging the days other than six per week will work injury to the
patients, diminish the chances of proper attendance, crowding the work into fewer days and thus
hurrying it, and that there would not be any appreciable saving of money.
2nd That it is the earnest desire and purpose of the Staff to enable us to reduce the expenses of the
Dispensary by adopting such rules for the prescribing of medicines as to save at least 25 or 30% of the
present expense. The Staff is willing to be subject to a trial of say three months to prove to us that the
expense of drugs + medicine used in the Dispensary will be so diminished.
In view of the above the Committee would respectfully report, that in their opinion the wisest course for
the present is, to keep the Dispensary open on every day, excepting Sundays and Holidays.

Dear Dr.,
I am very sorry that you have misunderstood my statement regarding the non attendance at the
dispensary of some of the newly appointed members of the staff. I have made no charge and did not
intend to make any against anyone connected with the dispensary. I simply stated to you officially what
the President of the hospital desired me to state – nothing more nor less. What action you should take
in the matter, whether you should bring the

matter to the notice of the supervising committee of which you are the chairman – it is, of course, not
for me to say. I have, I think, stated in my letter to you that I have been activated by the most friendly
feelings towards every one connected with the dispensary and that I find no fault with, nor have I the
right to control, the attendance of any one connected with the institution. I am sorry, very sorry indeed,
that you have misunderstood me and the object I had in view when I made the communication

to you referred to above. As the secretary of Med. Staff of the dispensary, I thought it my duty to report
to you briefly what transpired at our meeting, held at your house on the 8th inst. since you, sometime
ago, desired us to communicate everything pertaining to the working of the dispensary and which
required the action or the attention of the Board of Directors to you and not to the Directors; I have
endeavored to comply with your instructions and, to my astonishment, you now find fault with me for
doing what you wished me to do!

In regard to the nominations made by us to fill the 2 existing vacancies, as I understand it, the
Supervising Committee and the Board of Directors have to ratify the elections before the gentleman
named can be appointed. I need hardly add that you can reject them if you choose, and we will send in
other names. According to the rules which have been accepted by all the 3 Boards, their concurrence Is
needed to make an appointment in the Dispensary Staff.

Dear Sir,
The Board of Directors begs to acknowledge the receipt of your esteemed favor which
contains your resignation as Attending Physician at Mt. Sinai Hospital. –
On this occasion, the Board begs to express its appreciation of the great services you have
rendered Mt. Sinai Hospital, for upwards of 24 years. –
The consciousness that you have accomplished so much good in our charitable institution
by the vast amount of physical suffering you have relieved, and, above all, the devotion you have
lavished upon the sick children, (whose sufferings appeal the more to our inmost sympathies on
account of their helplessness), must ever be a source of pride to you; we, on our part, feel with
much satisfaction, that you, dear Sir, are one of those who have assisted in making our Hospital
what it is to-day.
Our regret at accepting your resignation as attending physician is made the less sharp by
the hope, which we entertain, of seeing you one of our 2 consulting physicians, positions which we
have recently created, but which are as yet unfilled.

appointment – in which he asks me to say to you that he is very desirous indeed of securing the
appointment. Is there anything for him to do in the matter before he returns (on Saturday, Sept.
25.)?

Rules and Regulations
for the
Staff of the Out-Door Department
of Mt. Sinai Hospital
I. The staff of the Out-Door Department shall, for the present, consist of fourteen physicians,
attending to the four departments already established: Internal Pathology, External Pathology,
Gynaecology [sic], and Paediatrics [sic].
II. New departments may be created when necessary.
III. The Staff of the Out-Door Department shall hold regular meetings at intervals, and in
accordance with rules to be determined by its own members.
IV. At the last regular meeting of each year, the staff shall elect from among its members, a
President and a Secretary, which officers shall attend to the internal business of the staff, and represent
it in all its relations with the Medical Board, and with the Board of Directors, during the ensuing year.
V. Special meetings of the staff of the Out-Door Department may be called at any time by its
President, in compliance with a request, either from any two members of the staff, from the Secretary
of the Medical Board, or from the Secretary of the Board of Directors.
VI. It shall be the duty of the members of the staff of the Out-Door Department to attend
regularly at their respective classes, or in case of unavoidable absence, to procure a substitute, for a
period of not over a week, immediate notice of which shall be given to the Supervisory Committee.
VII. The members of the staff shall use the greatest economy possible, in prescribing medicines
for the sick in the Dispensary.
VIII. Requisitions for instruments, or apparatus, or suggestions for the management of the work
of the Out-Door Department, shall be made at a regular meeting of the staff, and submitted by the
Secretary to the Supervisory Committee of the Medical Board.
IX. If any charges or complaints are made against members of the staff of the Out-Door
Department, they shall be submitted to the President, who shall either bring them personally before the
member in question, or before a meeting of the entire staff, for its action in the matter. A majority vote
of the members of the Staff shall, if desired by them, be transmitted to the Medical Board for
ratification or rejection. It is by this means, if necessary, that the resignation of a member shall be
requested.
X. Resignations shall be sent in writing to the Secretary of the Out-Door Staff, and if accepted by
that staff transmitted to the Secretary of the Supervisory Committee of the Medical Board for
ratification. Applications for appointment to the staff of the Out-Door department may be made either
to the Secretary [handwritten correction: Secretaries] of the Board of Directors, of the Supervisory
Committee of the Medical Board, or of the Staff of the Out-Door Department. All applications favorably
considered by the two bodies first named shall be sent to the Out-Door Staff for opinion, and such
names as are then favorably returned to the Supervisory Committee of the Medical Board shall be
considered by them for appointment, and their selection reported to the Board of Directors for
confirmation.
XI. All official communications, of any nature whatsoever, from any member of the Out-Door
Staff to the Board of Directors, or the Supervisory Committee of the Medical Board, must be made
through one of the officers of the Out-Door Staff to the Secretary of the Supervisory Committee of the
Medical Board.
XII. Amendments or additions to these rules may at any time be suggested in a meeting of the
staff of the Out-Door Department, and if accepted after suitable discussion by two-thirds of the
members of the staff, may be sent to the Medical Board, for ratification or rejection.
